By the Claims Postmark Editorial Team · Last updated: · Facts checked against official HPI and Harvard Pilgrim publications current in 2026

The Health Plans Inc claims address for most plans is a single post office box in Westborough, Massachusetts. Below you will find that address, the electronic payer IDs, the appeals and member-form addresses, the phone numbers, and a checklist for getting a claim to Health Plans, Inc. (HPI) on the first try.

Quick answer

Health Plans, Inc.
PO Box 5199
Westborough, MA 01581

Electronic claims: payer ID 04271 (HPI also lists 44273).

Check the member ID card first. HPI administers plans for many different employers, and it says the claims address and payer ID can vary by plan and network. If the back of the card shows a different address or payer ID, the card wins.

HPI uses one post office box for nearly all incoming mail: provider claims, provider appeals, member appeals and member reimbursement forms. What changes is the attention line and the form that travels with it. Each card below shows the address exactly as HPI prints it on the relevant page or form. The last card is the corporate headquarters, included for reference only.

HPI payer ID for electronic claims

HPI tells providers they can send claims electronically using the Harvard Pilgrim Health Care payer ID 04271 or the payer ID 44273, which HPI labels as its WebMD ID. Harvard Pilgrim's April 2026 payer ID reference guide confirms the routing from the other side: when a member ID begins with HH, the claim goes to payer ID 04271, and the guide shows the HPI name next to that destination. Independent payer directories also list "Health Plans Inc - HPI" as an alternate name under 04271, and a clearinghouse payer list shows "Health Plans Inc" under 44273.

Which one should you use? Use the payer ID printed on the card if there is one. Otherwise, 04271 is the ID both HPI and Harvard Pilgrim publish, so it is the safer default for an HH member ID. Our guide to the HPI payer ID and electronic claims covers clearinghouse naming, ID formatting and the most common rejection reasons.

Health Plans Inc phone directory

These are the organization's official contact numbers as HPI and Harvard Pilgrim publish them. HPI runs dedicated phone lines for many employer clients, so the number on the back of the card is usually the fastest route.

How to mail a claim to Health Plans Inc correctly

Most paper claims that bounce do so for ordinary reasons: the wrong payer, a mistyped member ID, or a missing field. This checklist puts the steps in the order a billing office actually works.

  1. Confirm eligibility for the date of service. Providers can check through HPI's provider portal using the 9-digit patient ID that begins with HH, or by calling the Provider Services number on the card.
  2. Read the back of the card. Copy the claims address and payer ID exactly as printed. If the card names another network or carrier for a service (for example dental), follow the card.
  3. Use the right form. CMS-1500 for professional services, UB-04 for facilities, and the ADA form for dental. Typed or printed forms scan more reliably than handwriting.
  4. Fill in the fields claims systems check first. Full patient name, date of birth, member ID, dates of service, diagnosis and procedure codes, charges and units, the rendering provider's NPI and the billing tax ID. Harvard Pilgrim's manual lists these as the core of a "clean" claim, and the same logic applies to any payer.
  5. Enter the member ID without dashes or spaces. Watch the difference between the letter O and the number zero.
  6. Attach the other insurer's EOB if HPI is secondary. A coordination-of-benefits claim without the primary payer's explanation of benefits is likely to be denied or pended.
  7. Address the envelope to the PO Box. Use the three lines shown below. Do not add the West Park Drive street address; HPI does not publish it for claims.
  8. Keep proof of when you sent it. Save a ledger printout or mailing record with the date. If a timely filing denial ever comes back, that record is what an appeal needs.
  9. Follow up through the portal. Search the claim by patient ID or claim number. If nothing appears after a reasonable processing period, call Provider Services before your filing window closes.

Members: filing your own claim with HPI

Most of the time your doctor or hospital files the claim. You normally only file yourself when you paid a provider directly, used an out-of-network provider who would not bill the plan, or have a benefit that works by reimbursement (some plans include fitness, weight-loss or travel reimbursement).

  • Form: HPI's Member Reimbursement Form, available from HPI's member forms page or your plan's HPI website. HPI also accepts reimbursement requests through the My Plan member portal.
  • What to attach: itemized bills or receipts showing the provider's name and full address, the dates of service, a description of each service, the full charge, and anything you already paid.
  • Who gets paid: the form asks whether you already paid (HPI reimburses you) or not (HPI pays the provider).
  • Before you mail: HPI cautions that some forms do not apply to every plan, so read your Summary Plan Description (SPD) first.

Filing limits and appeals at a glance

Timely filing depends on the network

HPI does not publish one deadline for every plan. It tells providers to check the member's participating provider network for filing limit terms, and its provider appeal form says the prevailing network's filing limit applies. For claims under the Harvard Pilgrim network, Harvard Pilgrim's 2026 manual sets 90 days from the date of service. Details and worked examples are in our HPI timely filing limits guide.

Appeals use the same PO Box, with a form

Provider appeals go to PO Box 5199 with HPI's provider appeal form, one form per claim, plus the EOP and the claim. Member appeals go to the Member Appeals Department at the same box. Federal rules for employer group health plans give members at least 180 days after a denial to appeal; your SPD may allow longer. See the Health Plans Inc appeals address and process.

Who Health Plans, Inc. is, and why the card matters

Health Plans, Inc. is a third-party administrator (TPA). It does not sell insurance policies to individuals. Employers, unions and public bodies that self-fund their health benefits pay the claims themselves and hire HPI to run the plan: enrollment, ID cards, claim processing, customer service and appeals. According to HPI's history page, the company was founded in 1981, became part of Harvard Pilgrim Health Care in 2005, and today describes itself as a subsidiary of Point32Health. Its headquarters is in Westborough, Massachusetts.

Because every employer's plan is set up separately, HPI pairs plans with different provider networks (Harvard Pilgrim, UnitedHealthcare, Cigna and others appear on HPI cards). That is the reason the claims address and payer ID can vary, and why the card is the first thing to check. Our guide to reading your HPI ID card walks through each part of the card.

Map shows HPI's corporate headquarters (official address published by HPI). Claims and appeals go to the PO Box, not this building.

Frequently asked questions

What is the Health Plans Inc claims address?

For most plans administered by Health Plans, Inc. (HPI), paper claims go to Health Plans, Inc., PO Box 5199, Westborough, MA 01581. HPI notes that the address can vary by plan and network, so the address printed on the back of the member ID card takes priority.

What payer ID does Health Plans Inc use for electronic claims?

HPI lists two electronic payer IDs: 04271, the Harvard Pilgrim Health Care (HPHC) payer ID, and 44273, labeled as the WebMD payer ID. Harvard Pilgrim's own payer ID guide routes member IDs that start with HH to 04271. If the card shows a different payer ID, use the card.

Do appeals go to the same address as claims?

Yes, for HPI-administered plans. HPI's provider appeal form and its member appeal form both use PO Box 5199, Westborough, MA 01581. Use the correct form for your situation, because HPI returns incomplete provider appeals without processing them.

What phone number should I call about an HPI claim?

Start with the number on the back of the member ID card, because many employer plans have a dedicated line. HPI's general toll-free customer service number is 800-532-7575, and its Westborough headquarters line is 508-752-2480.

What is the timely filing limit for Health Plans Inc claims?

HPI says filing limits are set by the member's participating provider network, so there is no single HPI-wide number. For claims under the Harvard Pilgrim network, Harvard Pilgrim's provider manual sets 90 days from the date of service.

Is Health Plans Inc the same company as Harvard Pilgrim?

No. HPI is a third-party administrator of self-funded employer plans and a subsidiary of Point32Health, which is also the parent of Harvard Pilgrim Health Care. Harvard Pilgrim's own claims box in Quincy, MA is not the address for HPI-administered plans unless the card says so.

Can I mail claims to HPI's street address in Westborough?

HPI publishes its PO Box for claims, appeals and member forms. The 1500 West Park Drive street address is the corporate headquarters. Use the PO Box or the address printed on the ID card for anything claim-related.

Is this the official Health Plans Inc website?

No. Claims Postmark is an independent guide and is not affiliated with or endorsed by Health Plans, Inc. HPI's official website is hpitpa.com.
